Technical Assistance:
Please keep all hazardous substances in a locked place - even though the Teen Center is a shared space, all cleaning products must be in a locked place during day care hours.

Comments:
An unannounced monitoring inspection was conducted with center staff on February 27, 2019 to determine compliance with licensing standards. There were 37 school age children present with seven staff actively supervising. The children were arriving on the bus, playing in the gym, doing homework, playing Dr. Seuss "Jeopardy", and eating afternoon snack. Snack served today: cucumbers and yogurt. Five children's files were reviewed - please see Supplemental Summary for missing items. The last shelter in place drill was conducted 9/17/2018. One medication for one child was reviewed and in compliance. Violations:
Standard #: 22VAC40-185-130-A
Description: Based on review of five children's records, the facility failed to have documentation of an immunization record for each child prior to enrollment. Evidence: Child 1 did not have documentation of an immunization record prior to enrollment.

Plan of Correction: Center staff will request the immunization record from the parent.

Standard #: 22VAC40-185-140-A
Description: Based on review of five children's records, the facility failed to have documentation of a physical within the first days of enrollment. Evidence: Child 1 did not have documentation of a physical within the first 30 days of enrollment.

Plan of Correction: Center staff will request the information from the parent.

Standard #: 22VAC40-185-550-D
Description: Based on review of the emergency drill log, the facility failed to have documentation of an evacuation drill each month. Evidence: there was no documentation of an emergency evacuation drill in November 2018 or December 2018.

Plan of Correction: Evacuation drills were resumed 1/7/2019. An evacuation drill will be conducted tomorrow for February.
