Consultation was given on care of children with biting, transitions, calming techniques and rest time.
An unannounced renewal visit was conducted today from 10:00-1:00. All rooms were in ratio with direct care staff. Children were observed in free play, infant care, transition to snack, hand washing, diapering and transition to outside play time. Reviewed staff records, asbestos management plan, emergency preparation, background checks, trainings and administrative policies and procedures. Standard #: 22VAC40-185-160-C Description: Based on record review, an independent contractor (IC) does not have a current TB screening within the last two years. Evidence: IC's most current TB was September 2016 and would have expired September 2018. Plan of Correction: Center Director will request an updated TB screening from the Independent Contractor.
Standard #: 22VAC40-185-250-B Description: Based on asbestos management plan, a visual review of areas where asbestos was detected has not been completed as required every 6 months. Evidence: An asbestos review was completed in April 2018 and the next was not completed until February 2019. A review should have been completed October 2018. Plan of Correction: The Director will work with the church to schedule and maintain documentation of reviews every 6 months as required by the management plan.
Standard #: 22VAC40-185-550-L Description: Based on record review, a sample of 10 injury forms are incomplete. Evidence: 3 of 10 injury forms do not document when the parent was notified. Plan of Correction: The Center Director will redo injury form to ensure the time is documented.
