An unannounced renewal inspection was conducted today from 10:20am-3:00pm. There were 51 children (infants-5yrs.) supervised by 10 direct care staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing with push toys, creating "Mr. Sun" paintings, learning rhyming words during circle time, and playing outside on the playground. Bottle-feeding, diapering and hand-washing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-160-B Description: Based on record review, 1 of 5 staff records did not contain an acceptable form of documentation of tuberculosis screening. Evidence: Staff C's (date of hire: 1/24/2019) TB skin test contained documentation of the date the test was given, but did not include documentation of the results of the test. Plan of Correction: Will have it in the file by Thursday.
Standard #: 22VAC40-185-40-J Description: Based on record review and staff interview, the center did not maintain injury prevention procedures that were updated annually based on documentation of injuries and a review of the activities and services. Evidence: Staff F confirmed that the center did not have documentation of injury prevention procedures that were updated annually. Plan of Correction: It will be done by Friday.
Standard #: 22VAC40-185-60-A Description: Based on record review, 3 of 5 children's records did not contain all of the required information. Evidence: 1. Child #2's (start date: 1/2/2018) record did not contain documentation of either of the two required emergency contact persons, and parental review of emergency contact information. The most recent documented parental review of emergency contact information was dated 12/10/2017. 2. Child #3's (start date: 2/6/2017) record did not contain documentation of parental review of emergency contact information. The most recent documented parental review of emergency contact information was dated 2/2/2017. 3. Child #5's (start date: not documented) record did not contain documentation of the child's first day of attendance, and complete addresses for both emergency contact persons. Plan of Correction: We will give the parents notices and give them 10 days to return all missing information.
Standard #: 22VAC40-185-280-B Description: Based on observation, 2 of 6 classrooms contained hazardous substances that were not kept in a locked place using a safe locking method that prevents access by children. Evidence: The Toddler 1 (16-24 months) and Early Learners (36-42 months) classrooms had cabinets above the diaper changing stations that were unlocked and contained chemical cleaning solution spray bottles. Plan of Correction: We will put cabinet locks on.
Standard #: 22VAC40-185-500-B Description: Based on observation, 2 of 4 diaper changing classrooms did not contain a diaper storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Evidence: The Toddler 1 (16-24 months) and Toddler 2 (24-36 months) classrooms contained broken foot-operated trash cans. Plan of Correction: I have ordered them and they should be delivered by Wednesday.
