Comments:
An unannounced monitoring inspection was conducted today from 10:15am-1:00pm. There were 97 children (infants-5yrs.) supervised by 17 direct care staff. The physical plant, 4 staff records, 6 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed having circle time, measuring, practicing letter identification, stacking apples, reading books, and participating in gym classes. Bottle-feeding, diapering and hand-washing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-60-A
Description: Based on record review, 2 of 6 children's records did not contain all of the required information. Evidence: 1. Child #2's (start date: 10/30/2017) record did not contain documentation of an annual review of emergency information performed by the parent. The most recent documented parental review of emergency information was dated 9/23/2017. 2. Child #3's (start date: ) record did not contain documentation of an annual review of emergency information performed by the parent. The most recent documented parental review of emergency information was dated 2/29/2016.

Plan of Correction: We will have our parents review, update, and sign the registration forms.

Standard #: 22VAC40-185-510-A
Description: Based on a review of medications, 1 of 3 prescription medications did not have parental authorization. Evidence: Child #2's (start date: 10/30/2017) Albuterol had a medication authorization form that was not signed by the parent.

Plan of Correction: We will have the parents provide a signature on the authorization form.
