An unannounced monitoring inspection was conducted today from 10:20am-12:45pm. There were 35 children (infants-5yrs.) supervised by 14 direct care staff. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed practicing with tools, planting seeds, sewing, having circle time, coloring, and eating snack. Hand-washing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Description: Based on observation, 1 of 4 classrooms was not maintained in a clean, safe, and operable condition. Evidence: The 3-year old classroom had areas of chipped and peeling paint on the lower halves of the blue walls.

Plan of Correction: We will talk to the church and let them know it needs to be fixed.

Description: Based on record review, the center did not implement a monthly practice evacuation drills. Evidence: There was no documentation of fire drills practiced for the months of November 2018 and January 2019.

Plan of Correction: We will be more cognizant to get the fire drills on the calendar.

Standard #: 22VAC40-185-550-M
Description: Based on record review, 5 of 5 injury reports did not contain all of the required information. Evidence: 1. One injury report did not contain documentation of the time that the was notified. 2. One injury report did not contain documentation of staff and parent signatures or two staff signatures. 3. Five injury reports did not contain documentation on how the parent was notified.

Plan of Correction: I will retype a new form and give it to the teachers. I will set a reminder to check injury reports before they are put in the book.
