Discussion was held on the topics of current enrollment, current staffing, and training for all staff. A CPR & First Aid course was attended and administered to all staff on 02/18/2019.
The following inspection took place between the hours of 8 am through 1:30 pm. Today, I met with the Staff of the ImagiNation Learning Center located in Chantilly, Virginia. At the start of this unannounced monitoring inspection there were a total of 83 children with 17 staff; good ratios in all 9 rooms. A total of 10 children?s files and 5 staff files were reviewed. The children were engaged in the following: diapering, hand washing, tummy time, infant sleeping, toy play, center activities within each room, story time, crazy hat day, music, dance, learning numbers, outside trip time in the center wagons, lunch, nap time, and other organized activities. Standard #: 22VAC40-185-130-B Description: Based on review, 1 of 10 children's files reviewed indicated that the center did not obtain documentation of additional immunization once every six months for a child under the age of two years. 1. On the date of inspection (02/26/2019), the most recent immunization documentation on file for Child #3 was dated - 08/21/2018. 2. On the date of inspection, based on the information on file, Child #3 was under the age of two years. Plan of Correction: An updated review shall be obtained and placed on file for Child #3.
Standard #: 22VAC40-185-140-A Description: Based on review, 2 of 10 children's files reviewed indicated that each child did not have a physical examination by or under the direction of a physician within one month after attendance. Evidence - 1. On the date of inspection (02/26/2019), the following children did not have the documentation of a physical examination available for review: Child #5, and Child #10. 2. The start date for Child #5 was listed as 01/21/2019. The start date for Child #10 was listed as 08/08/2016. Plan of Correction: Documentation of a physical shall be obtained and placed on file.
Standard #: 22VAC40-185-60-A Description: Based on review, 1 of 10 children's files reviewed indicated that not all of the required information was contained within each child's record. Evidence - 1. On the date of inspection (02/26/2019), the addresses for the emergency contacts listed for Child #5 were not available for review. 2. The start date for Child #5 was listed as 01/21/2018. Plan of Correction: The information shall be obtained and placed on file.
Standard #: 22VAC40-185-270-A Description: Based on observation, two pieces of equipment were not in a clean and/or safe condition. Evidence - 1. On the date of inspection (02/26/2019), within the Infant II's room, a red climbing mat showed areas of wear near its corners. 2. The changing pad within the Hippo's room, has a changing pad with two small area that expose the layer underneath. This same area consists of cross threads that could become saturated. Plan of Correction: Both items shall be removed and replaced. The red climber was already placed on reorder.
