An unannounced renewal inspection was conducted from 8:45am-12:45pm. At the time of entrance, 83 children were in care with 15 staff members present. Children were observed during breakfast time (cereal, banana and milk), diaper changes, hand washing, playing with toys, singing, engaged in group activities, playing with ipads and listening to the teacher read a book. Interactions between the children and staff were positive. A selection of staff and children records, the physical space, evacuation drills, attendance records and the Emergency Preparedness Plan were reviewed. Standard #: 22VAC40-185-140-A Description: Based on record review, the center did not obtain a copy of a child's physical examination within one month after attendance. Evidence: 1. Child #3 began at the center on 7/27/2018 and the center did not have documentation of a physical in the child's file. Plan of Correction: The child is scheduled for a doctors check up today and have requested a copy from the parent.
Standard #: 22VAC40-185-270-A Description: Based on observation, areas and equipment of the center inside were not maintained in a safe and operable condition. Evidence: 1. In the Preschool classroom, the Pre-K classroom, the Two's classroom, the Infant classroom and the School age classroom all have chipped paint near the windows. 2. The Pre-K classroom left window ledge was rough underneath. 2. In the School age classroom, the sofa is not clean. 3. In the Infant Classroom, the rocking chair cushion had a large tear in the fabric. Plan of Correction: We will put in another work order to have the window seals sanded and painted. I will purchase a sofa cover, so that it can be washed. I will purchase a new cushion for the rocking chair.
Standard #: 22VAC40-80-120-E-2 Description: Based on observation, the findings of the most recent inspection of the facility was not posted. Evidence: 1. The center had the August 2018 Subsidy Summary posted inside a frame, but did not have the monitoring violation notice or summary report posted from August 2018. Plan of Correction: I will make sure I follow the posting requirements.
