A Subsidy Health and Safety inspection was conducted on 02/22/2019 to review supplemental health and safety requirements. The children were observed playing outside, eating lunch and preparing for nap. A formal observation was completed with the preschool and toddler/twos classroom. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and the center's mini bus was inspected today.
Five children?s records and five employee records were reviewed during this inspection. The center administers medication; however they did not have any being given at this time. Three injury reports were reviewed.
The center's first aid kit and emergency supplies were inspected.

A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Standard #: 22VAC40-665-520-B-11 Description: Based on record review, the center did not ensure that two of five children's records contained a written allergy care plan for each child with a diagnosed food allergy. Evidence: The record of child #3 (start date: 6/19/18) and child #4 (start date: 3/13/17) did not contain documentation of a written allergy care plan. The children's records were reviewed by the inspector and both revealed that the children had a diagnosed food allergy. Plan of Correction: Will request parents to complete allergy plan with doctor and return by 3/1/19.
Standard #: 22VAC40-665-650-E Description: Based on review, the center did not ensure to have a written policy and procedure that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members. Evidence: The center's policies and procedures did not contain a written policy and procedure to ensure care by consistent staff. Plan of Correction: Will update policy and procedures to add above by 2/26/19.
