An unannounced Monitoring inspection was conducted during the program on March 1, 2019. The Inspector arrived in the program at 3:00PM. The children were observed in the cafeteria where they were served a snack and participated in an art activity. The children were observed also in the gym where they played group games. There were 4 staff members present with 23 children, the children's ages ranged for 4 to 11 years old. Staff were observed interacting with the children and provided direct supervision of the activities. As reported, 48 children are currently enrolled in the program. Dismissal procedures were observed at the end of the program day. A sample of 6 children's records was reviewed. Four complete staff records were reviewed. Documentation of the center's annual fire and health inspections was current. staff reported there are no medications in the center. The first aid kit, documentation of the practiced emergency drills and exit maps and emergency procedures were reviewed. Findings were reviewed with staff at exit. Standard #: 22VAC40-185-60-A Description: Based on a sample review of children's records, the Center failed to ensure the following information was complete in each record: 60.A.10. written agreements between the parent and the center as required by 22VAC40-185-90; 60.A.16. first and last dates of attendance. Evidence: One of six records (child #2) did not have documentation of a written agreement between the parent and the center. Five of six records were not complete with the child's first date of attendance (chldren 1,2,4,5,6) Plan of Correction: The Program Director will ensure that information is complete in all children's records.
Standard #: 22VAC40-185-70-A Description: Based on a sample review of staff records, one of 4 records did not contain signatures of whom verified the references. Evidence: Documentation of the two verified references for staff 3 did not contain a signature. Plan of Correction: Documentation of all references will be signed by whom called to verify the references.
