An unannounced monitoring inspection was completed on 3/1/19. There were 31 children present with two staff at arrival of the inspector and the third staff person arriving at approximately 3:25 pm. Five children's records and three staff records were reviewed. No medications at the site. The inspector arrived at the school at 3:15 pm and departed 4:30 pm. The children were observed during snack and during free time.
Standard #: 22VAC40-185-140-A Description: Based on record review and interview, the center failed to ensure that each child had a physical examination by or under the direction of a physician within 30 days after attendance if not submitted prior to enrollment. Evidence: 1. There was no physical examination in the record for child #2. The documented first date of attendance for child #2 was 10/30/18. 2. The director verified there was no physical examination in the record for child #2. Plan of Correction: The site director will have the parent submit the child's most recent physical.
