Comments:
A Subsidy Health and Safety inspection was conducted on 3/4/19 to review supplemental health and safety requirements. An unannounced monitoring inspection was completed on 3/4/19. There were 38 children present in four groupings. Two as needed medications were reviewed. The inspector arrived at the center at 10:00 am and departed at 3:30 pm. The children were observed during free time in the classrooms, during lunch, and during nap. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on interview, the center failed to ensure that there was a written policy and procedure that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members. Evidence: 1. The director was asked about the policy to ensure consistent staffing. The director stated that she did not have this completed.

Plan of Correction: The policy will be developed in writing.

Standard #: 22VAC40-665-770-B-5
Description: Based on interview, the center failed to ensure that there were continuity of operations procedures to ensure that essential functions are maintained during an emergency. Evidence: 1. The director was asked for the written procedures for continuing operations in an emergency. The director stated that the procedures were not completed in writing.

Plan of Correction: The procedures will be developed in writing.
