Technical Assistance:
Recommended placing the most updated medication forms in the classrooms with the medications. Discussed concerns with the two's classrooms and the need for better organization.

Comments:
An unannounced monitoring inspection was conducted from 8:45 am-12:45. There were a total of 121 children with 21 staff members present. Children were observed during snack time, engaged in group activities, lunch and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The infant playground was resurfaced and the trees were removed from the playground. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review, the center has not obtained a immunization record before the child can attend the center. Evidence: Child #1, date of enrollment 2/25/19 does not have an immunization record on file.

Plan of Correction: The parents have been notified.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment of the center is not maintained in a clean, safe and operable condition. Evidence: There is a wooden play kitchen set in the Toddler 2 class that has a small hole that could cause a finger entrapment. In the Preschool 2 class, there is peeling and chipped paint on the far back wall in the classroom.

Plan of Correction: WE will fix the kitchen set and contact maintenance to paint the wall.

Standard #: 22VAC40-185-510-C
Description: Based on a review of medications, the center is not following the medication procedures for administration. Evidence: Child #2 long term medication form is not signed by the parent.

Plan of Correction: We will have the parent sign the form.

Standard #: 22VAC40-185-510-G
Description: Based on medication review, the center has medication that does not have a prescription label in accordance with the prescriber's instructions pertaining to dosage, frequency, and manner of administration. Evidence: Child #4 has a long term medication that does not have the pharmacy prescription label attached.

Plan of Correction: We will call the parent to get the pharmacy label.

Standard #: 22VAC40-185-520-C
Description: Based on observation and staff interview, the center has not followed the requirements for administration of diaper ointment. Evidence: There was no parent authorization to apply diaper cream for child #4, #5 and #6. Many diaper creams were found not labeled in the Twos, Preschool 1 and Toddler 1 rooms. There was a plastic container in the Preschool class that was not labeled.

Plan of Correction: We will review all diaper cream forms for all classrooms by the end of next week. We will discuss the maintenance of the diaper cream binder with all staff.
