An unannounced monitoring inspection was conducted from 9:30am-1:20pm. At the time of entrance, 19 children were in care with 6 staff members present. Children were observed counting odd numbers and reading off the word wall, playing with toys, listening to music and playing on the floor with the teachers. A selection of staff and children records, the physical space, emergency supplies, evacuation drills, attendance records and the Emergency Preparedness Plan were reviewed. Standard #: 22VAC40-185-70-A Description: Based on review of seven staff records, the center did not maintain complete staff records. Evidence: 1. Staff #5 (began 2/26/2019) and Staff #6's (began 2/25/2019) file were missing documentation that two or more references were checked before employment. Plan of Correction: Reference letters were emailed to Great Beginnings today 3/05/2019. Staff #5 furnished 3 references and Staff #6 furnished 3 references.
Standard #: 22VAC40-185-270-A Description: Based on observation, Areas and equipment of the center inside were not maintained in a clean, safe and operable condition. Evidence: 1. In the Cubs classroom, the paint is peeling on the wall to the right of the door and on the white lower trim on the wall to the right of the windows. 2. In the Penguin classroom, the paint on the door is chipping. 3. In the Lambs classroom, the paint trim is chipping and the pink paint near the shapes is peeling and chipping. There are 3 nails or bolts not flush with the wall. Plan of Correction: On-going repairs have been in Progress since February 16, 2019. Painting in the Cubs, Penguin and Lambs classroom are expected to be completed by March 23, 2019.
Standard #: 22VAC40-185-280-B Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. In the Cubs classroom, a febreze air freshner and two odor eliminators were on an open shelf above the sink in the bathroom. 2. In the Penguin classroom, a container of clorox wipes were on an open shelf above the sink in the bathroom. 3. In the Lambs classroom, a container of clorox wipes and 2 febreze air freshner cans were on an open shelf in the diaper changing room. Plan of Correction: The Cubs classroom is not in use since the September 2018. The Clorox wipes were placed in a locked Cabinet in the presence of the inspector on 3/5/2019. The clorox wipes and Febreze were placed in a locked cabinet in the presence of the inspector.
Standard #: 22VAC40-185-500-B Description: Based on observation, the center did not meet all diaper requirements. Evidence: 1. In the Lambs classroom, the changing pad had a small hole on the surface and was not considered a nonabsorbent surface for diapering or changing. Plan of Correction: The changing pad was replaced in the presence of the inspector on March 5, 2019. Diaper changing paper is used at each diaper change. The manufacturer's label states that it meets Diaper Changing regulations under code: ASTMf-2388 for approved surfaces.
Standard #: 22VAC40-185-520-C Description: Based on observation, the center did not follow all requirements when diaper cream is used. Evidence: 1. An open container with more than 3 diaper creams were found on the lowest open shelf of the changing table in the Penguin classroom and were accessible to children. Plan of Correction: The container lids were secured and placed on a high shelf well above the reach of the children in the presence of the inspector on March 5, 2019.
Standard #: 22VAC40-185-550-E Description: Based on record review and interview, the center failed to maintain a record of the dates of the practice drills for one year. Evidence: 1. The practice drill record on the wall had the drills for January, February and March 2019 only. The Center Director could not locate the 2018 practice drill record. Plan of Correction: At the time of the inspection, the past year's drills could not be located. The Center Director is a new hire. The Drill Chart for 2018 will be located and emailed to the inspector by March 15, 2019.
