Discussion was held on topics involving out-of-state background checks, requirements for documenting annual review of the emergency preparedness plan, and safe locking methods for hazardous substances.
An unannounced monitoring inspection was conducted today from 9:45am-1:00pm. There were 54 children directly supervised by 10 staff and all classrooms were in ratio. The physical plant, the outdoor playground, 5 staff records, 6 children's records, 3 medications, evacuation drills, emergency supplies, the first aid kit, injury reports and policies were inspected. Staff training hours were reviewed and staff are exceeding training requirements. Children were observed participating in various arts and crafts activities involving the theme of Book Fair Week at the center, practicing writing, decorating a Dr. Seuss hat, and making puppets. Lunch today was veggie soup, crackers, cheese, banana, and milk. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of materials for the children and the center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-60-A Description: Based on record review, 1 of 6 children's records reviewed were not complete. Evidence: 1. Child #1's record (Date of enrollment: 6/6/2016) does not contain documentation of an annual update of emergency information. Child #1's record was last updated on 5/27/2016. Emergency information is required to be updated annually by the parents to confirm all information is correct and current. Plan of Correction: We will obtain requested records from parent today as well as implement a time of the year to request the update of emergency contacts every year.
Standard #: 22VAC40-185-550-E Description: Based on record review and staff interview, the center did not maintain documentation of the dates when shelter-in-place drills were practiced. Evidence: 1. The center did not have documentation of dates when shelter-in-place drills were practiced. 2. The center director stated the shelter-in-places drills were practiced but they did not have documentation of the dates. Plan of Correction: We will implement the requested log to keep track of when the plan has been updated
Standard #: 63.2(17)-1720.1-B-3 Description: Based on record review and staff interview, a Child Abuse and Neglect Central Registry search was not completed within 30 days of employment for an employee who has resided outside the state of Virginia within the last 5 years. Evidence: 1. Staff #4 (Date of hire: 8/27/2018) has resided outside the state of Virginia within the last 5 years as indicated on the Sworn Disclosure Statement signed on 8/13/2018. Staff #4's record does not contain documentation that an out-of-state Central Registry search was initiated. 2. The center director stated that the out-of-state Central Registry search was not initiated on this staff member because she searched online and found that the state of California prohibits the release of Child Abuse and Neglect Registry information. Plan of Correction: We will have the employee complete the CPS form for California today and mail it out by the end of the week.
