A Subsidy Health and Safety inspection was conducted on 3/6/2019 to review supplemental health and safety requirements. Thirteen children were in care with three staff. Four staff records and five children's records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: 22VAC40-665-650-E Description: Based upon staff interview, the facility has not ensured that a written policy and procedure was developed and implemented describing how the vendor will ensure that each group of children receives care by consistent staff or team of staff members. Evidence: 1. Staff 1 stated that a written policy/procedure was not yet developed. Plan of Correction: The facility responded with the following: A consistency of care plan has been discussed and will be put into writing and shared with all staff.
