Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 9:15 AM and departed at 12:15 PM. Fourteen children were in care with three staff. Morning activities were observed. No children's injuries were recorded since June 2018. It was reported that no medication is administered to children in care. Four staff records and a sample size of five children's records were reviewed. An exit meeting was conducted with the program director prior to closure of the inspection.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based upon review of five children's records, the facility has not ensured that each child has a physical examination by or under the direction of a physician before attending or within one month after attending. Evidence: 1. The record provided for child 4 indicated that the child began attending on 12/31/2018. Results of a physical examination were not in the child's record. 2. Staff 1 stated that child 4 began attending and then was absent for a period of time due to a family situation and that the family has not been unable to obtain a doctor's appointment yet.

Plan of Correction: The facility responded with the following: This has been addressed with the parent who has scheduled a physical examination but has to go through the military facility who does not an appointment available for a couple months.

Standard #: 22VAC40-185-500-A
Description: Based upon observation, the facility has not ensured that children's hands are washed with soap and water or a discposable wipe after eating. Evidence: Child 6 was observed eating breakfast at the small table inside the window. When child 6 finished eating, he got up from the table and joined in the children's activities without washing his hands,.

Plan of Correction: The facility responded with the following: The child arrived late/after breakfast was served. The parent asked if we could serve him breakfast. In the future, we will ensure that children's hands are washed directly after eating.

Standard #: 22VAC40-185-500-B
Description: Based upon observation, the facility has not ensured that diapers are disposed in a leakproof or plastic-lined storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Evidence: Staff 1 was observed changing a diaper. The lid on the diaper pail was not on the diaper pail as the connection for foot operation was broken.

Plan of Correction: The facility responded with the following: The diaper pail had just broken and will be replaced.
