Comments:
The licensing inspector initiated an unannounced complaint inspection in the center on February 13, 2019, continued on February 14, 2019 and completed on March 5, 2019 in response to a complaint received by the licensing office regarding staff qualifications/training and staffing/supervision. Review of center documentation, interviews with staff members and observations were conducted. Based on information gathered during the investigation, a preponderance of evidence does support the allegation regarding staffing/supervision therefore the complaint is determined to be VALID. Description: Based on staff interviews, the licensee did not ensure children under 10 years of age are always within actual sight and sound supervision of staff. Evidence: Center staff, staff #1 and staff #2 confirmed child #1, age 2 was left in the center's hallways by the classroom teachers. Child #1 was in the hallway outside the two year old classroom for approximately one to two minutes around 3:55 pm on February 7, 2019.

Plan of Correction: The center director will have a sight and sound supervision training and will be practicing hands offs with all of the staff tonight and it is mandatory for all staff. The two staff members involved that were the child's caregivers are going to receive disciplinary action accordingly.
