Comments:
An unannounced renewal study was conducted today from 2:45pm-5:30pm. A total of 70 children were in the care of 12 staff. Children were observed transitioning from naptime, during handwashing, snack [yogurt and crackers], storytime and outdoor play. Infants were observed during bottle feeding, and diapering. A sample of 12 children's records, 4 children's records, emergency supplies, medications, and other documents were reviewed. Areas of non-compliance are documented on the violation notice. Description: Based on a review of records, the center did not obtain parent confirmation of an annual review for 3 out of 12 children's records. Evidence: The most recent annual review documented for: 1. Child #4 was obtained 2/22/2017 2. Child #5 was obtained 12/20/2017 3. Child #7 was obtained 11/17/2017

Plan of Correction: We will make sure the parents return the paperwork annually.

Description: Based on observations, a complete first aid kit was not kept on each floor of the center. Evidence: A first aid manual was missing from the first aid kit on the second floor.

Plan of Correction: Corrected during inspection; A manual was printed and placed upstairs.
