Description: Based on review of shelter-in-place practice drills, the center failed to ensure that at least two drills were held annually. Evidence: 1. The last documented dates for shelter-in-place practice drills were 12/22/2017 and 09/05/2017. 2. Staff verified that those were the last documented dates for a shelter-in-place drill.

Plan of Correction: The current program director started in November 2018 and did not realize that shelter-in-place drills had not occurred for 2018. Dates have been placed on the calendar to ensure that the shelter-in-place drills occur this year.

Description: Based on a review of board member background checks, the center failed to ensure that current board members had completed a fingerprint background check by September 30, 2018. Evidence: 1. Board Member #1's background check was completed on 01/17/2019. 2. Board Member #2's background check was completed on 10/24/2018. 3. Staff verified that the background checks were not completed by September 30, 2018.

Plan of Correction: In the future all board members will complete a fingerprint background check within 30 days of taking office.
