Comments:
An unannounced monitoring inspection was conducted on Friday, March 8, 2019 to determine the center's compliance with licensing standards. The inspector arrived at 9:15 am and concluded the inspection at 12:45 pm. The center?s director and assistant director assisted the inspector throughout this inspection. The census for today?s inspection consisted of 108 children in the direct care of 19 staff. Upon the inspector's arrival, children were engaged in various activities. The preschool children were observed in their respective classrooms during circle time. The children in the Preschool #1 classroom were observed during a teacher-led activity where they reviewed the shape, number, color, and letter of the week. The letter of the week is ?Y,? the children participated in sounding out the letter and individually identifying words that start with the letter ?Y.? The children in the Pre-K #1 classroom were observed interacting with peer and staff. The children were observed during centers, while others prepared to go outside. Some of the children were observed eating a morning snack and lunch. Staff interacted with the children in a positive manner. The playground area was also inspected. The classrooms are decorated with the children's artwork and are equipped with age and stage appropriate materials. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, menu and various parent information. The center's first aid kit and non-emergency supplies were inspected and found complete. The center does not provide transportation at this time. Ten children's records and ten staff records were reviewed during today's inspection. Emergency medications and authorizations were reviewed. Health Inspection: 01/23/2019 Fire Inspection: 06/04/2018 The last documented evacuation drill was held on 2/05/2019 The last shelter-in-place drill was documented on 08/17/2018 The monthly menu was posted and today's lunch consisted of: Spaghetti, salad, bread sticks, bananas, and milk. Standard #: 22VAC40-185-140-A
Description: Based on a review of ten children's records on 03/08/2019, the center did not ensure one child had a physical examination by or under the direction of a physician. Evidence: 1) The record for Child #1 (DOE: 08/16/2018) did not contain a physical examination. Each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance.

Plan of Correction: Per the Director: We will contact the parent and have the pediatrician fax the physical. In the future, the center will monitor when physicals are due.
