A complaint was received on February 19, 2019 related to a parent being called to pick up a child because of rodents at the center. An unannounced joint complaint investigation was made with the local health department on March 4, 2019 in response to this complaint. Based on observations, interviews, and documentation reviewed, the complaint resulted in a violation of the Standards. The results of this complaint inspection were emailed to the center director on March 6, 2019.

Description: Based on observation, interviews, and review of documentation, the center failed to ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1. Observation of areas of the center revealed mice droppings/excrement in the following areas of the center: on the floor of two kitchen food storage closets, in a cabinet underneath the kitchen hand sink where cleaning products were stored, behind the refrigerator and freezer in the kitchen, on the floor in the closet of one classroom, in two places in one classroom accessible to children (by cots in one corner of the classroom and at the easel/stacked chairs in another part of the classroom by the windows), on the floor in the staff bathroom by the kitchen. 2. Five staff interviewed stated that mice have been seen both alive (running through a classroom) and dead (caught on multiple traps in various areas and dead, not on traps, in the staff bathroom and a closet). Staff stated that the mice had been noticed at the center for over a month. 3. There were sticky pads used for catching mice observed in areas of classrooms that were accessible to the children. 4. There was a violation cited by the local health department as a result of this investigation. The health department also provided the center with additional information regarding diseases that mice can carry and the risks of being exposed to the animal feces.

Plan of Correction: Pest control services had been provided on 2/5/19, 2/15/19, 2/27/19, and 3/1/19. According to the director, the areas of the center will be cleaned and the pest control service will continue on a monthly basis.
