An unannounced monitoring inspection was completed on 3/8/19. There were 29 children present from seven months through 10 years of age at the center in seven classes with 13 staff supervising and the site director present in addition. Five children's records and five staff records were reviewed. No medications were present at the center. The inspector arrived at the center at 10:30 am and departed at 1:45 pm. The staff records were reviewed from 2:15 until 4:00 pm at the central location and the inspection was exited at that time. The children were observed during free time in the classrooms, during gym time, during lunch, and during nap.

Standard #: 22VAC40-185-140-A Description: Based on record review, the center failed to ensure that each child had a physical examination by or under the direction of a physician within 30 days after enrollment. Evidence: 1. There was no physical examination observed in the record for child #4. Child #4 who is seven months old was enrolled on 12/10/18. Plan of Correction: The site director will follow up with the parent to get a physical examination report for the child.
Standard #: 22VAC40-185-50-A Description: Based on observation and interview, the center failed to ensure that all children's records were treated confidentially. Evidence: 1. There was a list of seven children with allergy information that was posted on the wall at the entrance door to the school-age classroom. According to the site director there is no permission from the parent to post this confidential information. The list contained food and health allergies of children enrolled. Plan of Correction: This was corrected during the inspection. The list was removed from the wall and will be kept in the classroom in a confidential manner.
Standard #: 22VAC40-185-60-A Description: Based on record review and interview, the center failed to ensure that each child's record was updated annually. Evidence: 1. The record for child #5 was reviewed. The last child information form was completed August 2016. 2. According to site director, there was no update form for child #5 for 2017 or 2018. Plan of Correction: The site director will send update forms next week.
Standard #: 22VAC40-185-280-B Description: Based on observation, the center failed to ensure that hazardous substances such as cleaning materials were kept in a locked place using a safe locking method. Evidence: 1. There was a closet observed in the school-age classroom that was unlocked and contained cleaning supplies (gallon of bleach, soap/water solution spray, bleach/water solution spray, etc) in the closet. There were nine school-age children observed in the classroom watching a movie at the time that the closet was unlocked. Plan of Correction: This was corrected during the inspection. Once the inspector made the staff aware of the unlocked closet, the closet was locked.
Standard #: 22VAC40-185-550-D Description: Based on record review and interview, the center failed to ensure that evacuation (fire) drills were practiced monthly. Evidence: 1. There was no documentation of a fire drill practiced for January 2019. 2. According to the site director, it was confirmed that the drill was not practiced. Plan of Correction: Drills will be practiced and documented monthly.
Standard #: 22VAC40-80-120-E-2 Description: Based on observation, the findings from the most recent inspection ("Violation Notice") of the facility were not posted. Evidence: 1. The "Violation Notice" from the April 2018 inspection was observed posted. The last inspection at the facility which resulted in violations was November 2018. Plan of Correction: The "Violation Notice" from today's inspection will be posted.
