Comments:
A Subsidy Health and Safety inspection was conducted on March 11, 2019 to review supplemental health and safety requirements. There were 56 children present with 4 staff. Children were observed playing outside with jump ropes, basketballs, flag football, ping pong paddles and birdies, tag and washing hands. Reviewed 4 staff files, 4 children's files, medications and authorizations, emergency evacuation log and supplies, attendance and physical plant. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Description: Based on staff record review, staff files were incomplete. Evidence: Staff#1 did not have documentation of completes Virginia Pre-Service for Child Care Centers available for review.

Plan of Correction: we will check to make sure it's not at another facility. If not, we will ensure she completes it.
