A Subsidy Health and Safety inspection was conducted on 03/11/2019 to review supplemental health and safety requirements. Present during the inspection were 30 children, ages 5 years-12 years, in the direct care and supervision of 4 staff. There was an additional support staff member present during the inspection. The inspector reviewed 5 children's records and 4 staff records during the inspection. The staff stated that there was no medication on-site. The children were observed in the following activities: returning from a walk, participating in large motor play in the gym, during lunch and swimming. The inspector reviewed the emergency plans and the emergency supplies during the inspection. The inspector discussed children's allergies with the staff. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations of the supplemental subsidy health and safety were found and are noted on the Violation Notice. Standard #: 22VAC40-665-500-F Description: Based on observation, review of document and discussion with the staff, the vendor failed to maintain a current written list of all children's allergies, sensitivities and dietary restrictions. Evidence: 1. The record for Child #1 has a documented allergy; Child #1's allergy was not listed on the current allergy list. 2. The record for Child #4 has a documented allergy; Child #4's allergy was not listed on the current allergy list. 3. The record for Child #5 has a documented allergy; Child #5's allergy was not listed on the current allergy list. Plan of Correction: The Child Care Coordinator will have the assistant review all children's records and update the list.
Standard #: 22VAC40-665-520-B-11 Description: Based on record review and discussion with the staff, the vendor failed to obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: The record for Child #2 has a documented and diagnosed food allergy. There was no a written allergy care plan on-site for review at the 03/11/2019 inspection. Plan of Correction: The Child Care Coordinator will get a copy from the site and place in the record.
