Comments:
An unannounced monitoring inspection was conducted from 11:00 am - 1:30 pm with the center owner/director and staff. There were 49 children, ranging from two years to five years of age, present with seven staff supervising. Children were observed participating in table work, center play, playing on the playground and eating lunch. Lunch served today was pizza, carrots, apple sauce and milk. Five children's files and three staff files were reviewed and in compliance. The emergency escape drill log was observed and in compliance. There were no medications to review. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, it was determined that not all areas of the facility were maintained in a safe condition. Evidence: There was insufficient pea gravel on the playground under and around the red climbing structure. Less than 1" was measured. To be resilient, 6" of pea gravel is needed.

Plan of Correction: We will keep the children off of the red climber until we get resilient surfacing fixed.

Standard #: 22VAC40-185-340-D
Description: Based on observation, and an interview with staff, it was determined that there was not at least one staff member who meets the qualifications of program leader in each grouping of children. Evidence: Staff C was observed alone in a classroom with five preschool children.

Plan of Correction: We will combine the two preschool groups until staff obtains group leader requirements.

Standard #: 22VAC40-185-560-K
Description: Based on observation, and an interview with staff, the facility failed to sanitize the tables as required. Evidence: staff in the upstairs preschool room sprayed Basic All Purpose Cleaner on the table before lunch and then used paper towels to dry the table. To sanitize is to " treat in such a way to remove bacteria and viruses from inanimate surfaces through using a disinfectant solution (i.e., bleach solution or commercial chemical disinfectant) or physical agent (e.g., heat). The surface of item is sprayed or dipped into the disinfectant solution and allowed to air dry after use of the disinfectant solution."

Plan of Correction: Retrain staff on sanitizing tables.
