An unannounced Renewal inspection was conducted in the Center on November 29, 2018. As reported, 31 children are currently enrolled in the program. Upon arrival, the Licensing Inspector observed 15 children and 3 staff present in the school cafeteria. The children's ages ranged from 5 to 10 years old. Based on staff interview, a total of 20 children had attended the program on this date. A sample of 6 children's records and 3 staff records were reviewed. Documentation of the annual fire and health inspections was current. A prescribed medication was observed in the center. The children were observed during free play. Standard #: 22VAC40-185-130-A Description: Based on a sample review of 6 children's records, the Center failed to obtain documentation that each child had received the immunizations required by the State Board of Health before attending the center. Evidence: Immunization records were not available for review in two of six records, children 1 and 3. Plan of Correction: Staff will contact parents to ensure immunization records are obtained. Documentation will be filed in the records.
Standard #: 22VAC40-185-140-A Description: Based on a sample review of 6 children's records, the Center failed to obtain documentation of a physical examination by or under the direction of a physician for each child. Evidence: Documentation of physical examinations was not available for review in the records of children 1 and 3. The children have been enrolled over 30 days. Plan of Correction: Staff will notify the parents and will request a copy of a current physical examination for record retention.
Standard #: 22VAC40-185-60-A Description: Based on a sample review of 6 children's records, the Center failed to ensure information as required by the Standards was complete in each child's record. Evidence: The first date of attendance was not indicated in the records of children 3, 4, and 5. The Mother's work phone number was not listed in the record for child 3. The agreement between the center and parent was not signed by the parent of child #2. Plan of Correction: Staff will review the record with the child's parent to ensure information is complete and current.
Standard #: 22VAC40-185-510-G Description: Based on a review of medication, the Center failed to obtain written parental authorization to administer a prescribed medication. Evidence: Written authorization for medication administration was not available for review for child a. Plan of Correction: Staff will notify the parent and will request a signed authorization from the parent.
