Technical Assistance:
Discussed the requirements if a fingerprinting background checks results are "undetermined."

Comments:
An unannounced monitoring inspection was conducted on March 7, 2019, from 9:41 am -11:45 am. Upon arrival to the facility, there were 11 children present with three staff members. The licensing inspector reviewed supervision, staff qualifications and trainings, nutrition, required posting, the physical plant and administration. The center is not administering medication at this time. The children were eating snack , freely play in the playroom and infants were sleeping. Exit interview conducted with the Program Director.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on 2 of 6 staff records reviewed, the licensee did not obtain documentation of a negative tuberculosis screening at least every two years. Evidence: During the inspection conducted on March 7, 2019, staff #5 most recent T.B. test was 2/6/2017 and staff #4 most recent T.B. test 12/19/2016. The tuberculosis screening was more than two years old.

Plan of Correction: Staff will have updated TB test in the next 5 business days.

Standard #: 22VAC40-185-60-A
Description: Based on record review, the license did not ensure each child enrolled contained all required documents in the record. Evidence: On March 07, 2019, child #3 record was missing one emergency contact address, child #4 record was missing father's address, phone number and employer and employer's phone number and child #5 record missing child's home phone number and work phone number.

Plan of Correction: The corrections were made during inspection.

Standard #: 22VAC40-185-290-3
Description: Based on observation , the licensee did not enusre on all electrical outlet there is protective covers. Evidence: During the inspection conducted on March 7, 2019, there was two electrical outlets with out protective covers in the playroom.

Plan of Correction: The director corrected while the inspector was present.

Standard #: 22VAC40-191-60-B
Description: Based on record review the licensee did not ensure a completed sworn statement or affirmation before they are employed or provide volunteer service. Evidence: During the inspection conducted on March 3, 2019, the record for Staff #2 most recent sworn statement or affirmation was complete in 2008.

Plan of Correction: In the next 5 days the sworn statement will be completed and will send to inspector.
