Comments:
The inspector conducted an unannounced renewal inspection from 11:15 until 3:10 p.m. There were 13 children in care with three staff supervising at all times and the director was also present. Children were observed during circle time, working on writing skills, washing hands before and after lunch and during rest time. Five children's records and five staff records were reviewed. Violations are identified on the violation notice. Please contact your inspector at 276-698-9981 with any questions.

Standard #: 22VAC40-185-160-C
Description: Based on record review, the center did not obtain repeat TB tests/screenings for staff members every two years. Evidence: 1. The inspector reviewed five staff records. One of five records (#1) had an overdue TB test. The last one was completed 1/23/2017.

Plan of Correction: The staff person has an appointment to get the TB test completed.

Standard #: 22VAC40-185-60-A
Description: Based on record review, the center did not contain all the required information in the children's files. Evidence: 1. The inspector reviewed five children's records. One of five records (#3) did not contain work numbers for the parents.

Plan of Correction: The director will get he information from the parents.

Standard #: 22VAC40-185-240-C
Description: Based on record review, staff did not complete 16 hrs. of annual training. Evidence: 1. The inspector reviewed five staff records. Two of five records (#3 and #4) did not have 16 hrs. of annual training completed.

Plan of Correction: Director will have all staff complete 16 hrs. of annual training in the future.

Standard #: 22VAC40-185-410-1
Description: Based on observation, center staff roughly handled a child. Evidence: 1. The inspector observed staff #3 holding a child by shoulders and pushing child down on the cot saying in a harsh tone "go to sleep". 2. Staff #1 stated she knew what she did was wrong and was too rough.

Plan of Correction: Director talked with staff person about the incident during the inspection. Director will provide a refresher training on child development and behaviors with staff person.

Standard #: 22VAC40-185-560-K
Description: Based on observation, the center did not sanitize tables before eating. Evidence: 1. The inspector observed a staff person wiping the tables with a cloth and the staff person said it had bleach on it.

Plan of Correction: The director will review with all staff about sanitizing tables.
