Description: Based on observations, it was determined that various fall zone areas of the playground do not have an adequate level of resilient surfacing. Evidence: There is not enough mulch at the bottom of the slide and beneath the two bar climbers. In these areas, the mulch has worn down to dirt.

Plan of Correction: We have already ordered mulch and it is scheduled to be delivered at the end of the month. We will rake the mulch from other areas of the playground until the additional mulch is delivered and spread where needed.
