- Technical Assistance:
An unannounced monitoring inspection was conducted on February 21, 2019 from approximately 3:15 PM to 3:50 PM, and March 7, 2019 from approximately 3:30 PM to 6:30 PM to determine the facility's compliance with licensing standards. Only ratios were inspected on 2/21/19. The staff to child ratios were observed as follows on 2/21/19: Toddlers 2:9, 2 year olds 1:7, 3 year olds 1:13, 4 year olds 1:9, School age 1:9. The staff to child ratios were observed as follows on 3/7/19: Toddlers 2:9, 2 year olds 1:6, 3 year olds 1:8, 4 year olds 1:8, School age 1:7. Lunch today consisted of pizza with cheese, carrots, apple slices, and milk. All areas used for child care were inspected. The facility has age appropriate materials and supplies. The following required information was posted; the License, most recent inspection notice, emergency numbers, evacuation map/procedures, daily schedules, menu, and various parent information. The first aid kits and non-medical emergency supplies were inspected and found complete. The inspector interacted with staff and children in care. Five staff records and five children's records were reviewed. Per administration, there is no medication being administered at this time. There are staff on site with current First Aid, CPR, MAT and DHO training. The most recent evacuation drill was documented on 2/7/19. Standard #: 22VAC40-185-160-C Description: Based on a review of staff records, the licensee did not ensure that one of five staff records reviewed contained documentation of an updated tuberculosis screening which is required at least every two years. Evidence: The record of Staff #4 showed a record of last having a tuberculosis screening on 12/28/2016. Plan of Correction: Administration will ensure staff gets the TB screening completed.
Standard #: 22VAC40-185-280-G Description: Based on observation and discussion, the center did not ensure that if hazardous substances are not kept in original containers, the substitute container shall clearly indicate their contents. Evidence: The licensing inspector observed a spray bottle located at the diapering area in the infant and toddler classroom which was not labeled. Staff indicated the contents of the bottle included bleach and water. Plan of Correction: Administration will ensure the container is labeled.
Standard #: 22VAC40-185-290-3 Description: Based on observation, the center did not ensure all electrical outlets contained protective covers. Evidence: The licensing inspector observed two electrical outlets in the two-year old classroom that did not contain protective covers. Plan of Correction: Administration will replace the outlet covers.
Standard #: 22VAC40-185-320-B Description: Based on observation, the center did not ensure that each restroom area provided for children shall be equipped with soap, toilet paper, and disposable towels or an air dryer within reach of children. Evidence: The licensing inspector observed there was no soap available in the boy's restroom. Plan of Correction: Administration will replace the soap in the bathroom.
Standard #: 22VAC40-185-330-A Description: Based on observation, the center did not ensure that playgrounds shall be located and designed to protect children from hazards. Evidence: The licensing inspector observed a climbing tunnel on the playground that was designed with multiple holes in the sides of the tunnel. The holes measured 4.5 inches in diameter and were appropriate to enable children to climb on top of the tunnel. The tunnel measured 25 inches in height. The tunnel is located at the edge of a hill that is approximately five foot in height with an approximate incline of 45 degrees. There is a sidewalk at the bottom of the hill. Additionally, there is a tree stump which is exposed by one to two inches adjacent to one side of the tunnel which is a hazardous obstacle. The climbing tunnel is sitting directly on the ground with no resilient surfacing. Plan of Correction: Administration will ensure the playground equipment is not in use until it meets the safety requirements.
Standard #: 22VAC40-185-330-B Description: Based on observation, the center did not ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards. EVIDENCE: The licensing inspector observed playground equipment described as an airplane seesaw with no resilient surfacing present. The seesaw seats reach a height of 3 foot. The seesaw seat on one side is located 4 foot from a sidewalk. The rear area of the airplane equipment, used for sitting or standing, reaches a height of 19 inches. The rear of the equipment is located 4 foot from a retaining wall. This playground equipment requires a fall zone free of hazardous obstacles and resilient surfacing that complies with minimum safety standards. Plan of Correction: Administration will ensure the playground equipment is not in use until it meets the safety requirements.
Standard #: 22VAC40-185-350-E-4 Description: Based on observations of the center's classrooms, the center did not ensure staff to children ratio requirements were followed. Evidence: On 2/21/19, in the three year old classroom, the licensing inspector observed a total of 13 children with one staff present. Staff indicated there was no aide or additional teacher in the classroom. The ratio should have been 1:10. Plan of Correction: Administration will ensure ratios are met throughout the day.
Standard #: 22VAC40-185-500-B Description: Based on observation, the center did not ensure the diapering procedures were followed. Evidence: The licensing inspector observed the trash can used for diapering in the girl's restroom was uncovered. The diapering area should have a leakproof, covered disposal system that is either foot operated or used in such a way that the staff member's hand nor the soiled diaper touches an exterior surface of the storage system. Plan of Correction: Administration will provide an appropriate disposal system for the diapering area.
