An unannounced monitoring inspection was conducted on 3/12/19 from 9:30am - 11:40am. When the Licensing Inspector arrived there were 51 children ages two years old to five years old in care with eight staff. Children were observed participating in various activities in the classrooms and eating lunch. Records were reviewed for five children and five staff. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Standard #: 22VAC40-185-160-A Description: Based on a review of five staff records, it was determined that the licensee did not ensure that each staff member shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted no later than 21 days after employment and shall have been completed within 12 months prior to or 21 days after employment starts. Evidence: 1. The record for staff #2 (date of hire 11/5/18) contained documentation of a negative TB screening that was dated 8/14/17. 2. Staff #6 reviewed the record for staff #2, and confirmed that the negative TB screening received was completed more than 12 months prior to staff #2 beginning employment. Plan of Correction: The facility responded: Bullfrogs and Butterflies, Inc. always ensure each staff member has a non-expired TB test prior to working with children. Staff #2 provided a TB test (not screening) dated 8/11/2017 and read with negative results on 8/14/17; Staff #2 state date being within the 2-year TB expiration window. Bullfrogs and Butterflies will change their hiring policy that all new hires will have to provide a new Tb screening prior to their employment start date. A new TB screening (3/15/19) has been provided by Staff member #2.
