Comments:
An unannounced monitoring inspection was conducted with the director beginning at 8:45 AM and ending at 10:43 AM. There were fifteen children in care during the inspection. The children were four and five years. The children were observed playing with blocks, coloring, and dressing up in the home living area. Five children's records were reviewed. Four staff records were reviewed and in compliance. There were no medications on site to be reviewed. The first aid kit, flashlight, and weather-band radio were observed and in compliance. The emergency escape drill log was reviewed and in compliance. The most recent emergency escape drill was documented as 2/12/19. The most recent shelter-in-place escape drill was documented as 2/15/19.The most recent fire inspection was dated 2/13/18. The most recent health inspection was dated 4/13/18. Standard #: 22VAC40-185-130-A
Description: Based on a review of documentation and staff interview, the center did not did not obtain documentation of immunizations for all children in care. Evidence: Child A's, start date 9/4/18, record did not contain documentation of immunizations prior to attendance.

Plan of Correction: The center will request the missing documentation from the parent. The center will allow 10 days for the parent to provide documentation.

Standard #: 22VAC40-185-140-A
Description: Based on a review of documentation and staff interview, the center did not did not obtain documentation of a physical examination for all children in care. Evidence: Child A's, start date 9/4/18, record did not contain documentation of a physical examination within one month after attendance.

Plan of Correction: The center will request the missing documentation from the parent. The center will allow 10 days for the parent to provide documentation.

Standard #: 22VAC40-185-280-B
Description: Based on observation and staff interview, the center did not ensure that all hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: In the preschool classroom, there was antibacterial cream and hand sanitizing foam on the table which was accessible to the children. There was three bottles of cleaning solution, a bottle of hair spray, and spray can of clear sealant, and three cans of shaving cream in the supply closet that was left open and accessible to children. All of the labels stated "keep out of reach of children".

Plan of Correction: The center will ensure that all potentially hazardous substances would be kept locked and inaccessible to children. The center staff will ensure that all supplies are put away after use and that the supply closet is kept closed and locked.

Standard #: 22VAC40-185-290-3
Description: Based on observation, not all of the electrical outlets had protective covers as required. Evidence: There were two outlets in the bathroom of the preschool room that did not have protective covers.

Plan of Correction: The center will ensure that the electrical outlets have protective covers on them. The janitorial staff will be reminded to replace protective covers after cleaning the area.
