Comments:
The licensing inspector conducted an unannounced monitoring inspection form 9:40am - 12:00pm. There were fifteen preschool children in care with two staff. The children were observed playing in the centers, playing on the playground and preparing for lunch. Two staff's records and five children's records were reviewed. Violations from the previous inspection were also reviewed and one violation was repeated. Areas of non-compliance are identified on the violation notice. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record reviews, the center failed to obtain all required information for the children's records. Evidence: 1. The inspector reviewed five children's records. One of five records reviewed (child record #1) listed that the child could not be picked up by the other parent; however, the appropriate legal paperwork was not in the record.

Plan of Correction: The teacher has contacted the child's parent and requested this information, but to date has not received it. The parent will be contacted again and this information will re requested.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to maintain the facility in a clean and safe condition. Evidence: 1. The inspector observed the outside play area. There are three holes in the ground next to the basketball court. The holes are large enough for a child's foot and could cause a child to fall or trip. The inspector observed the classroom. There are four small holes in the wall next to the block area. The filter in the return air vent in the floor is bent and covered with dust.

Plan of Correction: The teacher will complete a maintenance request to have the repairs made and the filter changed. The children will not play in the area of the playground with the holes in the ground until the holes are filled.
