A complaint investigation was initiated on March 14, 2019 in response to a complaint received by the Fairfax Licensing Office on March 12, 2019. Interviews were conducted, records were reviewed, and observations were made in regards to the complaint citing sanitation procedures and communicable disease communication. There was not a preponderance of evidence to support the allegation and the complaint was deemed not valid. One non complaint violation was cited during the investigation and is identified on the in the Violation Notice.

Standard #: 22VAC40-185-280-G Complaint related: No Description: Based on observation, hazardous substances not kept in original containers were not labeled in a way that clearly indicated their contents. Evidence: A bottle of spray cleaner kept in the teacher's cabinet in the 4's class was not labeled in a way to identify the contents. Plan of Correction: We will begin to label the bottles to identify the contents when they are filled.
