An unannounced monitoring inspection was completed on 03/14/2019. Present during the inspection were 25 children, ages 5 years-12 years, in the direct care and supervision of 2 staff. There was a volunteer on-site assisting with homework. The inspector reviewed 5 children's records, 2 staff records and 4 medications during the inspection. The children were observed in the following activities: arriving to the program, during homework time, afternoon snack and playing outside. Proper hand washing and table sanitization were also observed. The inspector reviewed the emergency evacuation plans, the shelter-in-place plans and the emergency supplies during the inspection. The inspector discussed program lead requirements and subsidy with the staff. The inspector arrived for the on-site inspection at 2:30 p.m. and departed at 4:45 p.m. If you have any questions, please contact Becky Forestier at 540-309-2835.

Description: Based on observations and discussion with the staff, the facility failed to ensure that hazardous substances, such as cleaning materials, shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: There was an unlocked storage closet that contained a bottle of Goo-Gone on a shelf in the unlocked storage closet.

Plan of Correction: The Goo-Gone was moved and locked up.

Description: Based on record review and discussion with the staff, the facility failed to ensure that all employees shall undergo background checks in accordance with the Code of Virginia and every five years thereafter. Evidence: The most recent search of the central registry in the staff record for Staff #4 was dated 04/29/2011; there were not checks completed every five years thereafter.

Plan of Correction: Staff #4 will complete the paperwork tomorrow.
