Comments:
An unannounced mandated inspection took place between the hours of 11:20am-1:10pm. There were no children in attendance due to parent-teacher conferences. A sample of 5 children?s records and 5 staff records were reviewed. The physical plant, playground, evacuation drills, injury report logs, and first aid kit were also reviewed. Standard #: 22VAC40-185-160-C
Description: Based on record review, 1 out of 5 staff members was missing a follow-up tuberculosis screening. Evidence: 1. Staff #4 had a TB test that was dated 12/16/2016. There was no documentation of a more current TB test in the file.

Plan of Correction: Staff #4 has been asked not to return until TB test has been completed.

Standard #: 22VAC40-185-70-A
Description: Based on record review, 2 of 5 staff records were incomplete. Evidence: 1. Staff #3 was missing the name, address, and phone number of an emergency contact. 2. Staff #4 was missing the name address, and phone number of an emergency contact.

Plan of Correction: Staff members have completed emergency contact information.

Standard #: 22VAC40-185-550-M
Description: Based on record review, 1 of 3 minor injury records were incomplete. Evidence: 1. 1 of 3 injury records was missing 1 of the 2 required signatures from a staff member and parent or two staff members and the future action to prevent injury.

Plan of Correction: All staff has been reminded via email to fill form completely w/2 signatures.
