Comments:
An unannounced inspection was conducted to investigate a complaint received on 2/5/19. The complaint was related to the lack of sight and sound supervision of school age children as they were being transported to their elementary school on 1/31/19. Interviews were conducted with staff, and training and transportation documents were reviewed. Based on the preponderance of evidence, the complaint is determined to be valid. Complaint related: Yes
Description: Based on interviews and record review, children under 10 years of age were not always within actual sight and sound supervision of staff. Evidence: 1. On 1/31/19 Staff # 1 drove Child # 1, Child # 2, Child # 3 and Child # 4, school age children under the age of 10, to their elementary school and dropped them off. The center's van logs indicated that the children were dropped off at the center at 9:56am. 2. On 1/31/19, the elementary school had a two hour delayed opening due to inclement weather. On days with delayed openings, the school opens for parents and children at 10:20am. 3. Witness # 1 stated that on 1/31/19, as she arrived at the school she saw the center's van dropping off children at approximately 9:58am in the school's parking lot which is also the "Kiss and Ride" area. Witness # 1 stated that when the children were dropped off, there was no school staff present to receive them. Witness # 1further stated that after the children were dropped off, they ran to the back side of the building where the school's playground is located. 4. A school representative stated that school staff who assists the Kiss and Ride students, position themselves in the Kiss and Ride area approximately 5-10 minutes before the doors open. On 1/31/19, because the school opened at 10:20am, they would have been in position between 10:10am to 10:15am. 5. The children were not within sight and sound supervision for approximately 20-24 minutes of staff from the time they were dropped off in the school parking lot at 9:56am until the school opened at 10:20am.

Description: Based on staff interview and record review, the center did not maintain responsibility for children being transported until released to persons designated by the parents. Evidence: 1. On 1/31/19 at approximately 9:56am Staff # 1 drove 4 school age children under the age of 10 years to their elementary school and dropped them off in the parking lot approximately 24 minutes before the school opened on that day. 2. On 1/31/19, the elementary school had a two hour delayed opening due to inclement weather. On days with delayed openings, the school opens at 10:20am for parents and children. The center?s van logs indicated that Child # 1, Child 2, Child # 3 and Child # 4 were dropped off at 9:56am at the school. 3. Witness # 1 stated that on 1/31/19, as she arrived at the school, she saw the center's van dropping off children at approximately 9:58am, and there were no staff from the school present to receive them. 4. A school representative stated that the school does not allow children to be dropped off before the scheduled open time, and that staff designated to assist with Kiss and Ride students position themselves in the Kiss and Ride parking lot approximately 5-10 minutes before the schedule drop off time, which on 1/31/19 was 10:20am. When the children were dropped off at 9:56am, there was no one from the school present to assume responsibility for these children.
