- Technical Assistance:
Technical Assistance was offered regarding the following: Background checks, the lock on the door in the 2's bathroom and children's records, nap time and cot layouts.
An unannounced monitoring inspection was conducted from 10:10 am to 1:30pm. There were 31 children present 7 staff members. The children were observed playing with sensory tables, toys, creating collages with newspaper cutouts, eating lunch and taking naps. 5 staff and 5 children's records were reviewed. Emergency drill records were reviewed. Areas of non compliance are listed in the violation notice. Standard #: 22VAC40-185-130-B Description: Based on record review, the center did not obtain additional immunizations every six months for children under the age of two. Evidence: Child # 1's most recent immunization records were dated August 2018. Updated and additional immunization records were not obtained after 6 months. Plan of Correction: We will ask the parents to bring in additional records.
Standard #: 22VAC40-185-140-A Description: Based on record review, the center did not obtain a physical exam for a child within 30 days of enrollment. Evidence: Child # 1's first day of enrollment was 8/28/18. Child # 1's had a print out of the after visit summary but not a complete physical exam. Plan of Correction: We will ask the parents to bring in a completed physical exam.
Standard #: 22VAC40-185-150-B Description: Based on record review, immunization records were not signed and dated by a physician. Evidence: Child # 1's immunization records obtained in August 2018 were print outs from the medical office, and were not signed by a physician. Plan of Correction: We will check for physician's signature or stamps and dates when accepting immunization records.
Standard #: 22VAC40-185-340-D Description: Based on observation and record review, each group did not have at least staff person qualified to be a program leader. Evidence: 1. In the 4's classroom, there were 10 children in the classroom with Staff # 2 who works as an assistant teacher. Staff # 2 did not have any documentation in her record to show that she qualified to be a program leader. 2. In the infant classroom, there were three children present with Staff # 3 who works as an assistant teacher. Staff # 3's record does not have any documentation to verify that she is qualified to be a program leader. Plan of Correction: Staff # 2 and Staff # 3 will be receiving training and may be considered to be lead teachers. A new staff person was hired for the infant classroom but never came to work, causing a disruption in staffing.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on record review, the center did not obtain fingerprint results for an employee before the first day of employment. Evidence: Staff # 1's first day of employment was 11/13/18. Staff #1's fingerprint results were dated 11/19/18, 6 days after the first day of employment. Plan of Correction: We will obtain fingerprint results before letting new employees begin employment.
