Licensing inspector arrived at 12:30 pm and departed at 2:00pm to conduct a licensing inspection . Upon arrival there were seventeen children in care with six staff. Additional staff were present to assist. Children were observed napping. The sample size consisted of 5 children records and 5 staff records .
Standard #: 22VAC40-185-140-A Description: Based on 1 of 5 children's records reviewed, the licensee did not ensure the records contained a physical examination. Evidence: During the inspection conducted on March 7, 2019, the record for child #1 ( start date 01/29/2019) did not have a physical examination. Plan of Correction: To ensure English and non-english parents provide all required documentation prior to child's first day of attendance.
Standard #: 22VAC40-185-60-A Description: Based on 1 of 5 children's records reviewed, the licensee did not ensure each child enrolled had a record which contained all required information. Evidence: During the inspection conducted on March 07, 2019, the following record did not contain all required documentation: Child #4 did not have a complete address for emergency contacts. Plan of Correction: To ensure all required documentation is obtain by parents upon enrollment of children.
