Technical Assistance:
Discussed new SHSI requirements.

Comments:
Licensing inspector arrived at 12:30 pm and departed at 2:00pm to conduct a Subsidy and health safety inspection. Upon arrival there were seventeen children in care with six staff. Additional staff were present to assist. Children were observed napping. The sample size consisted of 5 children records and 5 staff records .

Violations:
Standard #: 22VAC40-665-520-B-11
Description: Based on observation, the vendor failed to have a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: During the inspection conducted on March 7, 2019, child #5 had a doctor diagnosed allergy but did not have allergy action plan from the doctor.

Plan of Correction: To ensure all written allergy care plan are provided by parent upon enrollment of all children .

Standard #: 22VAC40-665-540-A
Description: Based on record review, the vendor failed to have documentation of TB prior to employment. Evidence : During the inspection conducted on March 7, 2019, Staff #2 did not have a TB test prior to her employment date 03/04/2019 and came in contact with children.

Plan of Correction: To ensure correct verbage on TB notice from Peninsula Health Department is written correctly .
