63.2(17) License & Registration Procedures

Comments:
An unannounced Monitoring Inspection (SHSI) was conducted on March 11, 2019 from approximately 1:15 pm until 3:45 pm. Upon entrance there were 23 children and 10 staff present with children. The record review sample consisted of 5 children's records and 4 staff records. Children were observed during nap time, preparing for and eating snack. Three medications were reviewed. The playground was inspected however, the resilient surfacing was not measured due to inclement wether. The results of the inspection were reviewed and verified with the Site Supervisor during the exit interview. Areas of noncompliance are noted on the violation notice. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the licensee did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin. Evidence: During the inspection on March 11, 2019 on the center's playground there was a white plastic tube with a cap. The plastic tube measured approximately 4 inches in diameter and was protruding from the ground approximately 2 inches high. This poses as a tripping hazard for the children.

Plan of Correction: EHS Director order mulch for the playground. Staff wsa notify to not use the playground until further notice. They will be using the small playground.

Standard #: 22VAC40-185-510-A
Description: Based on documentation review,observation and staff interviews, license did not ensure prescription and nonprescription medication shall be given only with written authorization from the parent. to a child: Evidence: During the inspection on March 11, 2019, a medication for child #1(Albuetrol) was administered on 10-24-18 at 12:00 pm and 4:00 pm and on 12-13-18 at 10:15 am with written parental permission. The center had not obtained written parental permission prior to administering medication. The center's documentation requesting parental permission had not been signed by the parent and this was confirmed by the center director.

Plan of Correction: Teacher call mom for her to fill up the parent authorization part of the medication form.

Standard #: 22VAC40-185-550-M
Description: Based on documentation review and staff interviews, license did not ensure the center shall notify the parent by the end of the day of any known minor injuries and included all required documents. Evidence: During the inspection on March 11, 2019, 14 injury reports were reviewed and 5 did not include the date of parental notification of injury and the time parental of notification of injury.

Plan of Correction: Teacher were notify to add time and date of teachers calling parents for accidents reports.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on 1 of 4 records reviewed , licensee did not ensure an Office of Background Investigation (OBI) fingerprinting determination letter stating eligible for hire was completed prior to staff beginning employment. Evidence: During the inspection on March 11, 2019, the record for staff #3 included documentation of training on 10-26-18 and first date of hire was 10-26-2018 and the OBI letter was dated 11-09-2018.

Plan of Correction: Center Coordinator explain EHS Director about background checks to be done prior staff to first working day.
