An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted on March 11, 2019 from approximately 1:15 pm until 3:45 pm. Upon entrance there were 23 children and 10 staff present with children. The record review sample consisted of 5 children's records and 4 staff records. Children were observed during nap time, preparing for and eating snack. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover the Subsidy Health and Safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. The results of the inspection were reviewed and verified with the Site Supervisor during the exit interview. Areas of noncompliance are noted on the violation notice. Standard #: 22VAC40-665-650-E Description: Based on interviews and documentation review, the vendor did not ensure to develop and implement a written policy and procedure that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members. Evidence: During the inspection on March 11, 2019, the center did not have a written plan to address consistent care by staff. Plan of Correction: Center Coordinator will up date policy and procedure assisting consistent care.
Standard #: 22VAC40-665-770-B Description: Based on interviews and documentation review, the vendor did not ensure the emergency preparedness plan included all procedural components. Evidence: During the inspection on March 11. 2019, the center's emergency preparedness plan did not include accommodations or special requirements for infants, toddlers, and children with special needs to ensure their safety during emergency lock down and shelter-in-place. The plan only included procedures to address evacuation accommodations or special requirements for infants, toddlers, and children with special needs. Plan of Correction: EHS Director will re-write the emergency preparedness plan including all procedural components.
Standard #: 22VAC40-665-770-B-5 Description: Based on interviews and documentation review, the vendor did not ensure the emergency plan included all required procedures. Evidence: During the inspection on March 11, 2019, the center's emergency plan did not include continuity of operations procedures to ensure that essential functions are maintained during an emergency. Plan of Correction: EHS Director will add required procedures to the emergency plan.
