Description: Based on staff statements and written documentation, staff failed to ensure the care and protection of one child. Evidence: 1. Staff #1 failed to verify the labeled name on a baby bottle. 2. Child #1 was given a baby bottle that belonged to another child and drank approximately 5 1/2 ounces of formula/breastmilk.

Plan of Correction: 1. Facility Director retrained all Infant staff teachers on American Day School?s infant feeding policies and procedures. These procedures were revised and updated on March 15, 2019. Updated procedures and policies were reviewed with all staff on March 15, 2019. 2. All staff* will complete the additional child health and safety training courses listed below by March 22, 2019: ? Age ? Appropriate Nutrition Feeding and Support Breastfeeding. ? Ensuring the Health and Safety of Infants and Toddlers in Childcare. ? Infection Control in Child Care Setting. 3. Facility implemented the following changes to feeding procedures effective March 18, 2019: ? Every morning the primary care giver will color code every breast milk bottle for the children in their care by placing a different color silicone band on every bottle containing breast milk. In the absence of the primary care giver the Lead Teacher will be responsible for this task. Each child will have a designated color. This silicone band will be labeled with the child?s full name, date, and the word ?breast milk?. Each child?s band color will be identified on the facility?s Feeding Daily Chart and will be displayed on the child?s personal cubby. At the end of the child?s feeding the primary care giver will remove the color band and proceed to clean and sanitize it for reuse the following day. These additional steps are currently in place to ensure that children are given the correct bottles. ? Facility implemented a procedure in which before feeding a child, the teacher must identify the bottle as formula or breast milk, then match the face of the child to the full name on the bottle?s label as well as match the color of the band to the child. The name label must be facing up and visible at all time while feeding the child. ? The Director and Lead Teachers will monitor to observe and be sure that these procedures are being followed. *Staff member involved in the incident was required to complete the additional training by March 16, 2019. Director received certificates of timely completion.
