An unannounced Subsidy Health and Safety Inspection was conducted on March 20, 2019 from approximately 7:25 am until 9:05 am. There were 38 children in care and 3 staff present with children. Additional children and staff arrived during the inspection. The total enrollment for the morning program is 70 children The record review sample consisted of 6 children's records and the on site required documentation for 4 staff records. Children were playing in centers to include dolls, legos, action figures, art and homework. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover the Subsidy Health and Safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. The results of the inspection were reviewed and verified with the Site Supervisor during the exit interview. Areas of noncompliance are noted on the violation notice. Standard #: 22VAC40-665-500-F Description: Based on documentation review and staff interviews, the vendor did not ensure to maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present. Evidence: During the inspection on March 20, 2019, there was not an allergy list available for review. There is a child, child #3 in the program with a diagnosed food allergy (fish, tree nuts and eggs) as verified from the center staff and documentation in the child's record. Plan of Correction: The center supervisor will ensure all staff are aware of the location of the most up to date allergy list.
