An exit interview was conducted with the director to discuss the results of today's inspection. Consultation was given regarding shelter-in-place and parent involvement requirements.
- Comments:

An unannounced renewal inspection was conducted today, March 15, 2019 to determine the center's compliance with licensing standards. The inspection was initiated at 12:50pm and concluded at approximately 3:30pm. Upon arrival, the following ratios were observed: five infants/ two staff, seven toddlers/ two staff, and six Pre K children (four 4 year olds and two 3 year olds ) with one staff. Children were observed eating snack consisting of goldfish crackers and strawberries, playing with toys, using the restroom/ diapering procedures, and interacting with staff. All areas used by children were inspected to include the main hallways, children's restrooms, classrooms, and cafeteria. Due to today's thunderstorm the children's playground was not inspected. The center had the required postings displayed on the bulletin board upon entering the center and in children's classrooms. The center's first aid kit and emergency supplies were inspected with the required supplies found. Five children's records and two staff records were inspected with the majority of the required information present, see attached violation notice. The following was also inspected on this day: annual inspections, fire drills, daily infant reports, injury reports, parent handbook/ agreements, and other pertinent information. Standard #: 22VAC40-185-60-A Description: Based on the inspection of the children's records, the required information was not listed. Evidence: During the inspection of five children's records, it was discovered child #2 and #4 did not have listed the telephone number of the parent's employer. Also child #5 was missing the name and telephone number of the child's physician, along with numerical address of second emergency contact & telephone number. Plan of Correction: Per director: I will get this information and update the child's record soon as possible.
Standard #: 22VAC40-185-70-A Description: Based on the inspection of staff records, the required information was not obtained. Evidence: During the inspection of five staff records, it was discovered staff #1 did not have any employment references in the record. Licensing standards require two or more references to be checked prior to employment with documentation maintained to review. Plan of Correction: Per director: I will get this information and update the child's record soon as possible.
Standard #: 22VAC40-185-550-M Description: Based on the inspection of three injury reports, the required information was not listed. Evidence: During the inspection of three children's injury reports, it was discovered child #2 did not have the date and time the parents were notified of injury. Plan of Correction: Per director: I will sit down with lead staff to make sure they have that information listed and I will have my substitute to review the reports.
