An unannounced Renewal Inspection was conducted today from 9:35am to 11:40am. There were 32 (preschoolers) directly supervised by 6 staff. The physical plant, 6 staff records, 5 children?s records, 2 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play and individual activities. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Standard #: 22VAC40-185-160-C
Description: Based on review, the facility did not ensure that staff were obtaining subsequent TB screenings at least every two years from the date of the initial screening or testing. Evidence: Reviewed 6 staff records and found that Staff 5 did not have documentation of a TB screening that was completed every two years. Last documented screening was dated 3/7/2017.

Plan of Correction: Staff will get that done on Monday.

Description: Based on review, the facility did not maintain Injury Preparedness Procedures that are updated at least annually based on documentation of injuries and a review of activities and services. Evidence: Reviewed the PWC Risk Report: the PWC Risk Report encompassed the entire Parks and Recs system and was not specific to the preschool program at Chinn Aquatics and Fitness Center, and a review of the specific licensed facility's injuries and accidents.

Plan of Correction: Will add that to the audit that we are periodically looking at and assessing our risk injury plan.

Description: Based on Review, the facility did not maintain documentation of required information in children's records. Evidence: Reviewed 5 children's records and found that 3, Child 1, Child 2 and Child 4 did not have documentation of parent work telephone numbers.

Plan of Correction: Will contact parents today to get that information.

Description: Based on observation, the facility did not ensure that hazardous substances are kept in a locked place using a safe locking method that prevents access by children. Evidence: Observed in Room 2, a bottle of Purell hand sanitizer, labeled "Keep Out of Reach of Children", accessible to children in a basket on the low counter.

Description: Based on observation, the facility did not ensure that electrical outlets have protective covers. Evidence: Observed in Room 2, an uncovered electrical outlet on a "power-box" with additional outlets, above the CD player.
