Comments:
An unannounced monitoring inspection was conducted on March 20, 2019 from 11:30am until 1:00pm. Upon arrival to the facility, there were 47 children present with 8 staff members. The children were observed participating in age appropriate activities, eating lunch, and hand washing. A sample of 5 children's records and 5 staff records were reviewed. The licensing inspector toured the facility and reviewed supervision, emergency supplies, practice drills, nutrition, physical plant, required postings, and administration. A violation was cited in the area of special services. An exit meeting was conducted with the facility director prior to the end of the inspection.

Standard #: 22VAC40-185-570-E
Description: Based upon observation the facility did not ensure that bottles of milk were not properly labeled with the a date. Evidence: 1. Bottles of milk used for Child #1 and #2 located in the refrigerator in the infant room were not properly labeled with the current date. 2. The director toured the facility with the inspector and acknowledged and confirmed the violation.

Plan of Correction: Make sure bottles are labled and dated.
