A Subsidy Health and Safety inspection was conducted on 3/11/19 to review supplemental health and safety requirements. There were 114 children in care, aged 4 months-5 years with 18 staff. Children were active in story time, music time, art time, outdoor time, play groups, personal hygiene, lunch time, and rest time. Staff and children files were reviewed, along with other documentation. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Description: Based on review of documentation and discussions with staff the vendor failed to maintain compliance with this standard as required. Non-compliance with this standard was previously cited on 11/26/18 and therefore is marked as a repeat violation. Evidence: There were no daily attendance sheets in the following rooms upon inspection at 10:15am on 3/11/19: 1. Both infant rooms. 2. Both toddler rooms. 3. Both 2 year old rooms. 4. The 2-3 year old room.

Plan of Correction: Daily attendance sheets had been completed in all rooms since the last inspection, however this morning the sheets for the younger side of the building had not been copied and distributed at the time of inspection. Each Friday, prior to the close of business for the day, all classes will have blank attendance sheets for the upcoming week to ensure compliance.

Description: Based on review of documentation and discussions with staff the vendor failed to maintain compliance with this standard as required. Evidence: Staff members 2-5 had not yet completed the 10 hour pre service training as required. The allowed 90 days to complete had since expired. Staff 2: Hire date 2/14/2018 Staff 3: Hire date 9/20/18 Staff 4: Hire date 10/25/18 Staff 5: Hire date 10/23/18

Plan of Correction: Staff will finish completion of the training as soon as possible and a copy of the certificate of completion will be placed in their file for future review. In the future all staff will complete this training within the allowed 90 days to ensure compliance.
