63.2(17) License & Registration Procedures
An unannounced focused monitoring inspection was completed on 3/22/19 as a part of the conditional license period. There were 22 children present at arrival and and additional 42 children arrived in three different groups from public school pick up. Five staff records were reviewed. One emergency medication was reviewed. The inspector arrived at the center at 1:35 pm and departed at 5:00 pm. The children were observed during nap, during outside time, during arrival from school pick up, during afternoon snack, during free time inside, and during departures. There was discussion with the director about the following: out-of-state central registry checks for staff who reside out of state, hand washing for school-age children, field trip processes for school-age out of school trips, the renewal process from the conditional license.
Standard #: 22VAC40-185-350-C Description: Based on observation and interview, the center failed to ensure that the staff to child ratio was maintained for children who were in a mixed age group. The ratio for school-age children from five years through eight years is 1:18 and from nine years through 12 years is 1:20. Evidence: 1. There were 22 children observed to arrive at 3:08 pm with one staff person from pick up from the public school. The staff person stated that there were children age five through 11 years old in this group. The staff person was the only staff person supervising this group of children until 3:18 pm when the second staff person with additional school-age children arrived. 2. There was one staff person observed with 19 children ages five through 11 years again from 3:28 pm until 3:35 pm when the second staff person went outside with a group of children. Plan of Correction: At 3:18 pm when the second staff person arrived, this was corrected. At 3:35 when a third staff person arrived, the second was corrected.
Standard #: 22VAC40-185-550-H Description: Based on review of the emergency documents for the vehicles, the center failed to ensure that potential shelters were listed for each site frequently visited or routes frequently driven by center staff for center business (field trips, pick up/drop off of children to or from schools, etc.). Evidence: 1. The emergency documents were observed on two vans and two busses used for transporting children. There were several routes observed for school pick up and drop off and for field trips. There were no shelter locations noted for any of the routes. Plan of Correction: The director will add shelter locations for each route as appropriate.
