Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and specific training certifications obtained were determined through an on site interview. A follow up review of Staff files shall be conducted on a future date through the YMCA administrative office.

Comments:
The following inspection took place between the hours of 3:30 pm through 5 pm. Today, I met with the Staff of the YMCA school-age after-school program held on site at Cool Spring Elementary school located in the town of Leesburg, Virginia. At the start of this unannounced monitoring inspection there were a total of 29 children with 4 staff; good ratio for this one group. A total of 5 children?s files were reviewed. The children were engaged in the following: large motor skilled play in the gym, bathroom breaks, hand washing, snack, games, homework time, and other organized activities. Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children. Evidence - 1. On the date of inspection (03/25/2019), 4 spray containers of cleanser was observed in an unlocked cabinet on stage. 2. While there was a lock on the cabinet door, it was not engaged.

Plan of Correction: This area, behind the curtain, was off limits to the children today. In the future, this cabinet shall be locked.

Standard #: 22VAC40-185-430-K
Description: Based on observation the provision made for an individual place for each child's personal belongings was not utilized. Evidence - 1. On the date of inspection (03/25/2019), at approximately 3:20 pm, the laundry bins used by the program for each child's personal belongings remained stacked up and to the side. 2. The children placed all of the their belongings in a pile spread out against the far wall of the gym.

Plan of Correction: The bins shall be set out for use tomorrow.

Description: Based on review, evacuation drills were not practiced on a monthly basis. Evidence - 1. On the date of inspection (03/25/2019), based on the drill log posted on the parent bulletin board, the practice drill for February 2019 was either never conducted or not recorded. 2. The space to record the practice drill for February 2019 was left blank.

Plan of Correction: Fire drills shall be practiced and then recorded on a monthly basis.
